The following THREE (3) forms of identification are required:

1) Photo I.D. (Driver's License, Proof of Age or Student Card)
2) Credit Card (VISA, DEBIT, MASTERCARD) or $160 cash deposit.
3) Proof of Address (Copy of Bank Statement, Lease Agreement, Telephone Bill and/or Gas Bill which shows your current address).
Please attach scans of each document, or bring them in next time you visit our store.

Please read the following Terms & Conditions of Membership

1) Not to copy, duplicate or make any alterations to the DVD.
2) Not to part with the possession of any DVD(s).
3) Not to show publicly or exhibit the DVD except for private home use.
4) To return the DVD at the end of the hiring period in an undamaged condition.
5) To be responsible for payment(s) of any late fee(s) incurred by the use of this membership, and for full replacement costs of any DVDs damaged or not returned to The Seventh Art for any reason.

The Seventh Art policy is to refuse rental of MA rated films to persons under the age of 15 and R rated films to persons under the age of 18.
